This policy is underwritten by New York Life Insurance Company, 51 Madison Ave., New York, NY 10010, under Group Policy No. G-30923-0 on Policy Forms GMR-FACE/G-30923-0.
New York Life Insurance Company is licensed/authorized to transact business in all of the 50 United States, the District of Columbia, Puerto Rico and Canada. Who is eligible for this insurance?
This policy is available exclusively to licensed attorneys with The State Bar of California under age 60, who may request coverage for themselves and their lawful spouses and domestic partners under age 60. In order to become insured, satisfactory evidence of insurability must be provided and the required premium must be paid. You can request coverage for your dependent children age six months through 25 years, for $5,000 each ($1,000 if under age 6 months).
A spouse or domestic partner who is also a licensed attorney with The State Bar of California is eligible for either licensed attorney or spouse coverage, but not both. If both licensed attorney and spouse are covered as members, neither may insure the other as spouse.

Spouse/Domestic Partner—$100,000 to $3,000,000 in $25,000 increments.
When does coverage end?
A licensed attorney's insurance will end the earliest of: the date this policy ends at the option of New York Insurance Company or the policyholder; the date insurance ends for the licensed attorney's class; the end of the period for which the last premium has been paid for such licensed attorney; the licensed attorney’s premium due date coinciding with or next following the date the licensed attorney attains age 80 (26 for a child); or the licensed attorney’s premium due date coinciding with or next following the date the licensed attorney ceases to be a member of the California State Bar and no longer meets the definition requirements of eligible member.
Dependent coverage will end the earliest of the date the licensed attorney’s insurance ends under the group policy; the date the group policy is changed to end dependents’ life insurance; the date the person ceases to be dependent; or the end of the period for which the last premium has been paid for the dependent.
When is the coverage effective?
Licensed Attorneys must give evidence of their insurability satisfactory to New York. Subject to the requirements below, licensed attorneys will be insured on the first day of the month coinciding with or next following the date New York approves the request.
A licensed attorney/spouse must be able to perform the normal activities of a person in good health of like age on the date NC residents (a person of like age) the insurance is to take effect. If not, such insurance will take effect on the day the licensed attorney/spouse resumes such activities.
A licensed attorney/spouse might be hospitalized on the date the insurance takes effect. If so, insurance will take effect on the day after discharge.

How does the Accelerated Death Benefit work?
The Accelerated Death Benefit option is available to help terminally ill insureds during a difficult and often financially challenging time. Under this provision, you may request a one time advance payment equal to 60 percent (or $250,000, whichever is less) of your or your insured spouse’s or domestic partner’s current life insurance coverage. Your premiums will not be reduced.
This money can be used to help cover high prescription drug costs … medical bills … outstanding debts … to help pay for experimental treatments … the cost of modifications to your home … or for a family vacation—the choice is yours.
To qualify, a terminally ill insured must provide New York with proof of terminal illness and anticipated life insurance company expectancy (required twelve months or less), as well as any other necessary medical information requested. Please note that the receipt of Accelerated Death Benefits may affect your eligibility for public assistance programs and may be taxable. Prior to applying to receive such benefits, you should consult with the appropriate social services agency and seek the advice of a qualified tax advisor.
Are there any exclusions?
After coverage is in force for two years, benefits are payable for death from any cause, anywhere in the world.
If a covered person commits suicide whether sane or insane within two years from the date insurance takes effect, the benefit will be limited to the premiums paid.
